* Farm Rescue requests a minimal logistical expense reimbursement to help offset the cost of mobilizing equipment, vehicles and volunteers.
All documentation submitted to Farm Rescue will be kept confidential and reviewed only by committee members for the purpose of determining eligibility. Please do not send original documents unless you are notified by Farm Rescue to do so since documents will not be returned to you.
The process of selecting farmers who will receive assistance is based on many factors, which include, but are not limited to; farm management, geographical, financial, logistical and weather related considerations. Farm Rescue prohibits discrimination in its program on the basis of race, color, national origin, sex, religion, age, political beliefs, sexual orientation or marital status.
All statements and documents are true and accurate to the best of my knowledge. I understand that any misrepresentation will void eligibility for assistance from Farm Rescue. I/we hereby release Farm Rescue and further agree to indemnify Farm Rescue, its officers, committee members, volunteers and agents acting in its behalf, and to hold the same harmless from and against all claims, demands and liabilities, actions and causes of action and loss or damage, whatsoever, arising out of this application for and receipt of assistance from Farm Rescue to me/us. This release and indemnity agreement shall be binding upon the undersigned and the heirs, successors and assigns of the undersigned.
