Optional Demographic Information

To better communicate the impact that we make through our work, we are interested in learning more about you, your family, your operations, and your legacy. Set forth below, please find several optional demographic questions. Farm Rescue will not use in any way your answers to these questions, or your decision not to answer these questions, in evaluating your application.

After submitting your online application, Farm Rescue requires some additional documentation to verify ownership, field layout, and illness/injury. These documents include:

• Producer Farm Data Report from your local Farm Service Agency (FSA-578)
• FSA maps with fields highlighted
• Letter from physician, if application is based on a medical illness or injury

All documentation submitted to Farm Rescue will be kept confidential and reviewed only by committee members for the purpose of determining eligibility. Please do not send original documents unless you are notified by Farm Rescue to do so since documents will not be returned to you.

The process of selecting farmers who will receive assistance is based on many factors, which include, but are not limited to the following: farm management, geographical, financial, logistical, and weather related considerations. Farm Rescue prohibits discrimination in its program on the basis of race, color, national origin, sex, religion, age, political beliefs, sexual orientation, or marital status.

All statements and documents are true and accurate to the best of my knowledge. I understand that any misrepresentation will void eligibility for assistance from Farm Rescue. I/we hereby release Farm Rescue and further agree to indemnify Farm Rescue, its officers, committee members, volunteers, and agents acting in its behalf, and to hold the same harmless from and against all claims, demands and liabilities, actions, and causes of action and loss or damage, whatsoever, arising out of this application for and receipt of assistance from Farm Rescue to me/us. This release and indemnity agreement shall be binding upon the undersigned and the heirs, successors, and assigns of the undersigned.
