Application
To apply for assistance from Farm Rescue, click here to complete a two-page application. (The application form allows you to type in your answers.) Once completed, print out the application, sign it, and fax or return by mail to Farm Rescue along with the required documentation.
Applications that are complete will receive priority consideration. An application is considered complete if it contains the following:
• FSA form AD-1026A
• Financial balance sheet/statement from lending institution
• FSA maps with fields highlighted
• Letter from physician if application is based on a medical illness or injury
