Application
To apply for assistance from Farm Rescue, click here to download a two-page application. Print it out, answer the questions, sign the application, and fax or return by mail to Farm Rescue along with the required documentation.
Applications that are complete will receive priority consideration. An application is considered complete if it contains the following:
• FSA - Producer Print form
• Financial balance sheet/statement from lending institution
• FSA maps with fields highlighted
• Letter from physician if application is based on a medical illness or injury
